Student Health Services is committed to upholding the highest standards regarding the confidentiality of health records. Records are stored and maintained in accordance with Federal, State, and TCNJ safety and retention regulations. Health records are kept for 10 years after the most recent visit. After the designated time, the records are destroyed.
Requesting Immunization Records: Most record requests we receive are for Immunization records. If you have graduated in the past 10 years your immunization record is stored in OWL and you can access it through logging into the system. If you are having issues or need other health records please follow the instructions below for “Requesting Health Records”.
Requesting Health Records: Print the Authorization for Release of Health Information, complete the form, and fax or email it to our office at health@tcnj.edu. Be very specific about what you want to be released. Sign and date the form to avoid delays in processing your request. Note that this request is specific to Student Health Services’ records and will not contain records you may have in Counseling & Psychological Services (CAPS), Alcohol and Other Drug Support Services, Anti-Violence Initiatives, Athletics, or other offices on campus (see below for more information). Allow 7 business days for processing of requests. If picking up your records in person, bring your Driver’s License with photo ID for verification purposes.
The Authorization for Release of Health Information can only be used to release existing health records (past and present). We do not honor authorizations from students providing an individual with access to clinical records that may exist in the future.
