All incoming international students are required to complete TCNJ pre-entrance health requirements as a condition of
enrollment and entry into college housing.

Required Health Forms
All forms listed below except the New Student Medical History are pdf files that can be viewed & printed out (but not filled out online). If you click on the form and it does not open, you may not have a pdf reader program installed on your computer. For free download of Adobe Reader, click HERE.

|FORM||ABOUT||HOW TO SUBMIT|
|Record of Immunization (pdf)||This is a required form that you print out and is completed, signed & stamped by your physician or registered nurse. No other form will be accepted.
It is expected that you receive ALL required vaccinations BEFORE coming to TCNJ and submit the record to Student Health Services by the due date. If you are just starting a vaccine series such as Hepatitis B, an extension will be given so that you can complete the shots when you get to campus. If a required vaccine is not available in your area such as pertussis, varicella or meningococcal meningitis A,C,Y,W-135, you can receive the vaccine in TCNJ Student Health Services but you must do so within 48 hours of arrival. Please note that meningococcal meningitis C or A vaccine is NOT acceptable. To schedule an appointment in Student Health Services, send an email to firstname.lastname@example.org. |Email the completed Record of Immunization to email@example.com by the due date along with your other health form on this list. Your health forms will be checked by the nurse and you will receive an email if you are missing a vaccine.|
|Tuberculosis (TB) Screening Questionnaire (pdf)||This is a required form that you print out and complete yourself.

|Physician’s Evaluation for Tuberculosis (pdf)||This form is required ONLY if you answered YES to one or more questions on the Tuberculosis (TB) Screening Questionnaire (above). Print out the form and take it to your doctor to be completed, signed and stamped. If you have a positive TB test, you must submit a chest x-ray reading IN ENGLISH showing that your x-ray is normal.
||Email this form to email@example.com by the due date along with your other health forms. Be sure to include your chest x-ray reading if performed following a positive TB test. This reading must be in English.|
|Meningococcal Disease & Vaccination (pdf)||This is a required form that you print out and complete yourself. Note that all students entering campus housing must receive Meningococcal Meningitis A,C,Y,W-135 vaccine as a condition of entry. Meningococcal Meningitis C or A vaccine is NOT an acceptable alternative. If this vaccine is not available in your area, you can receive the vaccine in TCNJ Student Health Services but you must do so within 48 hours of arrival or you will not be permitted to remain in college housing. |Authorization to Treat a Minor (pdf)||This form is required ONLY for students who will not be 18 years of age when they arrive on campus. Print out the form and have your parent or legal guardian sign the form.||Email this form to firstname.lastname@example.org by the due date along with your other health forms.|
|New Student Medical History||This is a confidential form about your personal & family medical history. It is important that you answer these questions truthfully and to the best of your ability. This information is only seen by Student Health Services’ physician, nurse practitioners, and nurses.||THIS FORM IS ONLINE.
Log into OWL, the Online Wellness Link, at https://tcnj.medicatconnect.com using your PAWS username and password. Click on Forms at the top of the page and then on New Student Medical History.
Skip the Tuberculosis (TB) Screening Questionnaire and the Meningococcal Disease & Vaccination Sections on this form.
When the form is complete, click SUBMIT.
You may not be able to complete this form until your application to TCNJ is complete and you are activated in the system. This should occur at least 2 weeks before you arrive at TCNJ.
MEDICAL EXEMPTIONS FROM REQUIRED VACCINATIONS
Students are not required to have any vaccinations that are medically contraindicated. To apply for a medical exemption, the student must provide Student Health Services with a currently-dated letter written and signed by their healthcare provider containing the following information:
- the name of the vaccine that is medically contraindicated,
- the specific period of time that the vaccine is medically contraindicated,
- the reason(s) for the medical contraindication based upon valid medical reasons as enumerated by the United States Department of Health and Human Services, Advisory Committee on Immunization Practices (ACIP). Applications for medical exemptions are reviewed by the Director of Student Health Services and the student will be notified of her decision via TCNJ email. If approved, the student will be exempted from the specific immunization permanently (in the case of a permanent medical condition) or temporarily until such time that the student’s medical condition permits vaccination.
The College of New Jersey may temporarily exclude a student with a medical exemption from vaccination from classes and from participating in college-sponsored activities during a vaccine-preventable disease outbreak or threatened outbreak. The decision to exclude a student with a medical exemption will be made by the College in consultation with the New Jersey Commissioner of Health.
