CONSENT, WAIVER AND RELEASE AGREEMENT TO USE STUDENT HEALTH SERVICES – DUE DATE: July 15
CCS Students who DO NOT HAVE a court-appointed legal guardian:
Prior to the start of EACH academic year, CCS students and their parents must sign and submit the CCS Consent, Waiver & Release Agreement for students with NO Legal Guardian (pdf) to Student Health Services. Once the form is complete, log into OWL at https://tcnj.medicatconnect.com/ . Click on the UPLOAD icon and follow the instructions to upload the “No Legal Guardian: CCS Consent, Waiver And Release” document. Without this agreement on file for the current academic year, Student Health Services’ nursing and medical staff cannot see the CCS student except in cases of emergency. Having this agreement on file allows the CCS student to make an appointment to be seen in Student Health Services by the nurse practitioner or physician in the same manner as TCNJ students.
It is important to note that Student Health Services is not a walk-in clinic or an emergency room. Appointments can be scheduled by calling the office at 609-771-2889. Students are expected to bring the following to their appointment: Health Insurance & Prescription card (an extra card can be requested from the insurance company by the subscriber), and a form of payment (e.g., cash, MasterCard, Visa, TCNJ GetIt Points) to pay for chargeable services such as rapid tests, crutches, etc -- see https://health.tcnj.edu/cost/ .
CCS Students who DO HAVE a court-appointed legal guardian:
CCS students who have a court-appointed legal guardian for whatever reason can be seen in Student Health Services only if accompanied by their legal guardian. In addition the legal guardian must sign the Consent, Waiver and Release Agreement for CCS Students WITH Legal Guardian (pdf) and submit it to Student Health Services. Exceptions are made in cases of emergency. Once the form is complete, log into OWL at https://tcnj.medicatconnect.com/ . Click on the UPLOAD icon and follow the instructions to upload the “Legal Guardian: CCS Consent, Waiver And Release” document.
Questions about the agreement should be directed to the CCS office who will then consult with the TCNJ Office of General Counsel.
